Where to buy

Remember, buy it at Amazon in paperback (now 13.99 plus s/h) or for Kindle (3.99 and no s/h).  Also,  buy the paperback here: CreateSpace.   Of course, you may find it other places, such as Barnes and Noble.  The Nook version is also 3.99 and looks good.  Just make sure you get the current revision of the book.  Check eBay because sometimes I will auction a copy starting $0.99.
